How to place a form in a page

Copy and past the text below and update the number on the end to match your new form. Make sure there is no styling in the text/code (bold, italic, etc.) Remove any hyperlinks from any of the text below too, or else you will get an error message. the form may take a little time to load.work if you just built it.

BUILDING FORMS SAMPLE: Form Title will appear on the form unless to set it not to in options. Please attempt to put most of the form information in the WordPress content instead of here, for best results.

Manually add text line above a check box if it looks like it does below.

You can add headings as needed to break up a form.



For file uploads — put NOTHING in the actual file upload title area and instead add a text line above it separately. NOTE: You must also remove the placeholder words "Edit this text"
